Join one of our Small Group Training programs—just 5-7 clients per group, training 2-3 times a week for 12 weeks with the same dedicated Personal Trainers. You’ll receive personalized attention, accountability, and support as you work toward your fitness and health goals. These 12-week programs are available during early mornings, evenings, and weekends. 

FORM FITNESS GROUP 1: Trainer – Daniel Lac 

COED – Open to all! 

Training Schedule: 

• Tuesdays | 6:00-7:00 PM 

• Thursdays | 6:00-7:00 PM 

• Sundays | 7:45–8:45 AM 

FORM FITNESS GROUP 2: Trainer – Daniel Lac 

COED – open to all! 

Training Schedule: 

• Tuesdays | 7:00-8:00 PM 

• Thursdays | 7:00-8:00 PM 

FORM FITNESS GROUP 3: Trainer – Tara Silvaggio 

Women only 

Training Schedule: 

• Mondays | 6:15-7:00 AM 

• Wednesdays | 6:15-7:00 AM

small group fitness PRICING OPTIONS 

3 Times Per Week Training (Group 1) 

Option 1: One-time payment – $850 + GST - To be paid on the day of  registration 

Option 2: 3 monthly payments of $325 + GST 

Payment Dates: 

1) December 28, 2025 (or time of registration, if after December 14)

2) January 25, 2026 

3) February 22, 2026 

2 Times Per Week Training (Group 2 & 3): 

Option 1: One-time payment – $575 + GST - To be paid on the day of registration 

Option 2: 3 monthly payments of $220 + GST 

Payment Dates: 

1) December 28, 2025 (or time of registration, if after December 14)

2) January 25, 2026 

3) February 22, 2026  

Need-to-Know Info: 

  • If you're a current studio member, your membership or class pass will be put on hold during your 12-week Small Group Training. 

  • If you miss a session, you’ll receive a drop-in class credit valid for any group fitness class at Fifty 5 or FORM—usable before the program ends on April 5.

  • If you find the Small Group Training is not the best fit for you, please cancel by emailing Emily at emily@fifty5fitness.com by January 18th to receive a  full refund.

Registration Information

- CURRENT FALL SMALL GROUP TRAINING CLIENTS: You will have “first dibs” at winter training spots, and can register from now until Monday, December 22nd . Please email me at emily@fifty5fitness.com to let me know which Small Group you would like to register for. I will send you back a confirmation email by December 23rd to confirm your spot in the Small Group. Note that you can stay in the same group as the fall, or try out a new group for winter! 

- NEW CLIENTS: Registration will open on Sunday, December 28th at 12:00 NOON on WellnessLiving. 

HOW TO REGISTER

1) Login to your WellnessLiving account on December 28th at 12:00 NOON 

2) Click on CLASSES 

3) SCROLL to the first week of the session (January 12th) and book yourself into your first session (GROUP 1 – JAN 13 @ 6:00PM, GROUP 2 – JAN 13 @ 7:00PM, GROUP 3 – JAN 12 @ 6:15AM). This will be a FREE booking. ONLY 7 SPOTS AVAILABLE! 

4) Once you have done this, Emily will send you out a WELCOME package via email and connect with you on whether your payment will be the one-time payment, or 3 monthly payments. 

5) Emily will send you out your first invoice. Once this is paid, you have secured your spot in Small Group Training! 

Spaces are extremely limited—just 5-7 max per group! If this is something you want to take part in this Winter, please register on December 28th!
