cancellation policy
No refunds will be granted for no-show reservations. Classes will be processed as a class attended for all reservations.
An 8-hour notice is required for cancellation of any class.
FORM FITNESS No-Show fee is $15 (for Unlimited clients), or loss of 1 class (all other memberships).
FORM FITNESS Late Cancellation (less than 8 hours before class time) is $10 (for Unlimited clients), or loss of 1 class (for all other memberships).
For 5 class packages there will be a deduction of one class from your class package if there are any remaining. If there are no classes left in your package a $15 no-show fee will be charged to your credit card on file. If there is no credit card on file, the $15 no-show fee must be paid upon your next visit.
For single Drop-In passes purchased, the pass will be considered “used” for the no-show.
Classes at FORM will be cancelled IF ONLY 2 clients are registered. Classes can be cancelled by FORM up to 4 hours before the class start time. Clients in the cancelled class will be notified via phone and email.
